Students and PhD students – foreigners:

1. Students and doctoral students who are citizens of a European Union Member State or a member state of the European Free Trade Agreement (EFTA) – parties to the European Economic Area Agreement residing in its territory may use healthcare under the same conditions as Polish insured under the condition current European Health Insurance Card (EHIC) issued in the country of origin.

2. Students and PhD students who are not citizens of a European Union Member State or a Member State of the European Free Trade Agreement (EFTA) – parties to the European Economic Area contract, who study in Poland and hold a valid Polish Card or have been recognized as persons of Polish origin within the meaning of the provisions of repatriation and are not subject to the obligation of health insurance for a different title, they can insure themselves voluntarily by entering into an appropriate contract with the National Health Fund. In such a situation, the health insurance contribution is paid by the Poznan University of Technology.

To insure health, you should:

  1. Go to the Wielkopolska Provincial Department of the National Health Fund (NFZ) in Poznań, ul. Grunwaldzka 158, tel. 61-850-60-00 (between 8:00 am and 4:00 pm) in order to sign an agreement for voluntary health insurance. Documents needed for a contract at the NFZ:
    1. a certificate from the dean’s office or the secretary’s office of doctoral studies on the status of a student or doctoral student,
    2. Polish Card or document confirming Polish origin within the meaning of the provisions on repatriation,
    3. passport,
    4. a document confirming the registration in the Wielkopolska Region,
    5. PESEL (voluntarily)
  2. With the Polish Card / document confirming Polish origin within the meaning of the provisions on repatriation and the contract with the NFZ or a document issued by the National Health Fund, you should report to the Employee Matters Department in order to complete further formalities for health insurance. If the application was filed with the National Healthcare Fund on the basis of a document from the NFZ, a contract with the NFZ should be delivered immediately after receipt.
  3. After completing the above-mentioned formalities, Poznan University of Technology will pay monthly premiums for health insurance.
  4. In order to benefit from free healthcare, you must complete the DECLARATION in any INCOME, which has a contract with the National Health Fund.

3. Students and doctoral students who are not citizens of European Union Member States or a member state of the European Free Trade Agreement (EFTA) – parties to the Agreement on the European Economic Area and who can not prove a valid Polish Card or a document certifying that they have been recognized as persons of Polish origin within the meaning of the provisions on repatriation, they can conclude an agreement for voluntary health insurance with the National Health Fund and pay subscription.

4. To insure health, go to the Wielkopolska Provincial Department of the National Health Fund (NFZ) in Poznań, ul. Grunwaldzka 158, tel. 61-850-60-00 (between 8:00 am and 4:00 pm) in order to sign an agreement for voluntary health insurance.

Needed documents:

  • a certificate from the dean’s office or the secretary’s office of doctoral studies on the status of a student,
  • passport,
  • a document confirming the registration in the Wielkopolska Region,
  • PESEL (voluntarily)
